Back to School Support Request

You can request term time support for your family by completing this form. Please give as much detail as you can to enable us to assess your request fully.

Please remember

✔ We will provide you with high street vouchers which can be spent at one of the high street shops to purchase school uniform, sports kit and books/equipment/stationery

✔ For educational school trips we will need the details of the provider / school so that we can pay them directly

When requesting support for transport and school meal provision please provide information about what it is you need and the full costs set by your school

✔ When requesting support for other essential equipment please provide information about what it is you need and the full costs involved

If the online form is not fully completed we may not be able to process your request for support

Perennial will only share your information with third parties if it is needed to progress your application, if it is in your best interests and when we have explicit consent from you to do so.

We use data processors who are third parties who provide services for us, with whom we have contracts in place. This means that they cannot do anything with your personal information unless we have instructed them to do it. They will not share your personal information with any organisation apart from us. They will hold it securely and retain it for the period we instruct.
