School Holiday Help Request
You can request school holiday support for your family by completing this form. Please give as much detail as you can to enable us to assess your request fully.
Please remember
✔ To fill in the details of your registered childcare or holiday activity/club provider as we will need to pay them directly
✔ If the form is not fully completed we may not be able to process your request for support
✔ We can help towards the additional costs of food during the school holidays for children who receive free school meals during term time if your child/ren will not be accessing childcare or a holiday activity. We will provide you with food vouchers which can be spent at one of the high street supermarkets.

Perennial will only share your information with third parties if it is needed to progress your application, if it is in your best interests and when we have explicit consent from you to do so.
We use data processors who are third parties who provide services for us, with whom we have contracts in place. This means that they cannot do anything with your personal information unless we have instructed them to do it. They will not share your personal information with any organisation apart from us. They will hold it securely and retain it for the period we instruct.
